Job Description:
A company is looking for a Case Manager RN to facilitate
healthcare delivery and support member wellness.
Key Responsibilities
Facilitate the delivery of healthcare benefits and information to
determine eligibility
Develop and implement health strategies and programs to promote
member wellness
Coordinate care through assessment, planning, and advocacy for
individuals and families
Required Qualifications
Active RN licensure in NV
Minimum 3+ years of clinical experience, preferably with Medicare
members
Minimum 2+ years of case management or home health care
coordination experience
Associate's Degree or Nursing Diploma required; Bachelor's Degree
preferred
Willingness to obtain multi-state RN licenses if needed
