We are currently hiring two full-time MDS RNs (Hourly) to join our team. These positions require every other weekend and some evenings.

Your Key Responsibilities
Manage the MDS process with precision, ensuring timely and accurate resident assessments.
Develop and implement individualized care plans in collaboration with an interdisciplinary team.
Actively participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
Maintain compliance with evolving MDS regulations while providing clear communication with residents, families, and team members.
What You Bring
Current RN license in good standing.
RAC-CT certification required.
Expertise in MDS 3.0, RAI processes, and individualized care planning.
Strong organizational and interpersonal skills, with a commitment to quality care.

This is a full-time, on-site position where your leadership and expertise will drive resident-centered care. Requires every other weekend and some evenings.
